Cost*
Medicaid pays 100% of all classes. Medicare patients must pay a 20% co-payment. You may be able to get a reduced cost based on your income and household size. We will file claims for patients with insurance. If the claim is denied, you will be billed. Call your insurance company before coming to class because you may need precertification by your physician for insurance to pay.
Individual Counseling $31.00 per 1/2 hour, $61.00 per hour
Nutrition (1/2 hour)
Insulin (1 hour)
Group Classes
Basics Class $70 (2-3hours)
Diabetes Self-Management Program: $350 (10 hours)
You will be asked to provide the following information:
- Employer
- Number of people in your home
- Your household income
- Type of insurance - you must bring your insurance, Medicaid or Medicare card with you
- Precertification number from your physician (if required by your insurance company)
